Download and print the pdf order form, read and sign disclaimer. Mail frame(s) with order form, prescription copy (if available) and payment info. If you choose to prepay online, please enclose a copy of your paid receipt with order number.
Orders are shipped via the United States Postal Service First Class Mail and Priority Mail®.
Orders returned for refund will be refunded to the same method of payment: product price, less initial shipping charges and merchant fees. To cover lab charges, custom prescriptions are refunded at half price.
Returns or Exchanges are accepted within 30 days of order shipping date. Free shipping on returns and exchanges within 30 days.
